Re: Freedom of Information Act Request
Dear IRS FOIA Officer:
Under the provisions of the Freedom of
Information Act, 5 USC § 552, I am requesting copies of:
-
The revenue agent's report, including all notes and
workpapers
-
Any and all statements made by me or by any third
party to the IRS relating to my audit
-
[identify other records as clearly and as
specifically as possible]
Before you fill the request, please inform
me of any fees for searching for or copying the records I have
requested, unless this amount is less than $50.
If all or any part of this request is
denied, please cite the specific exemption that you think
justifies the refusal to release the information and inform me
of the appeal procedures available to me under the law.
I would appreciate your handling this
request as quickly as possible, and I look forward to a
response within 10 days, as stipulated in Section
552(6)(A)(I).
Sincerely,
John Smith
